Mid-Year Report

Some colleges require students to submit a mid-year report of their grades. If a college you have applied to requests a mid-year report:

 

✅ Complete the attached form: Midyear Request Form 2023-2024.

✅ Make that completed form into a PDF.
✅ Complete the Google Form and be prepared to upload your completed Midyear Request Form.

 

Please note, not all colleges require a mid-year report or transcript. Please check with each college before submitting the Mid-Year Request to your counselor.
