Attendance Office Hours: 7:30 a.m. - 3:00 p.m.CLOSED to students during class time (please visit before school, during break, office hours, or lunch)

Students who need to leave school early because they are ill must check-out through the HEALTH OFFICE. Do NOT use this form for illness-related early check outs.
Students are required to properly check-in and check-out of the attendance office in addiiton to the parent/legal guardian filling out this form. Failure to check-in or check-out will result in the absence being coded as a TARDY or TRUANCY.
Please do not report full-day absences here. There is a separate form for full-day absences.
Simply filling this out does not excuse your student's partial-day (period) absence or tardy.
Late Check-in: Students are required to check-in at the attendance office to receive a pass to go to their class, even if they arrive during office hours or between passing periods. Failure to check-in will result in an "Unexcused Tardy" or "Period Truancy."
Early Check-out: Students are required to go to the attendance office to obtain and Off-grounds Pass to leave early based on the parent/legal guardian's note that is submitted here on this form. If the parent/legal guaridan does not submit this form, then the parent/legal guaridan will need to sign the student out in person at the attendance window. Failure to check-out will result in a "Period Truancy."
Period 8: Please do not submit a verification note for Period 8 absences. Period 8 is OFFICE HOURS. Students are responsible for checking-in to Office Hours through Aeries when they are present at school.
Please consider donating $67 per day for each student absence. This is the average amount MBUSD loses in school funding for every absence reported. We are currently projected to lose approximately $2.3M due to absences.
