CURRENT STUDENTS MUST REQUEST TRANSCRIPTS ON CAMPUS.

Former students and graduates need to complete the Transcript Request for Past Students form. Specify if you are requesting a Transcript or an Immunization record by noting how many copies of each you’re ordering in the bottom left corner of this form. The fee is $5.00 per copy requested and is payable by check or money order only. Additionally, you must include a stamped and addressed return envelope so that we can mail your transcripts back to you or the address of your choice. These forms are available on campus and orders may be dropped off in person, but campus pickup is no longer an option.

If you are ordering multiple copies, keep in mind that one #10 (business-sized) envelope with one first-class stamp will hold two sealed transcripts. If you are ordering three or more records, make sure to include enough stamped envelopes so that we can mail your records back to you.
